A Simple Technique of Central Venous Catheterization for Long Term Parenteral Nutrition in Neonates and Infants


Abstract
After "Wilson" et al (1962)42) described the technique of subclavian venipuncture for main-tenance of optional blood volume by monitoring central venous pressure, there have been dev-eloped many modifications of technique, and many kinds of instruments and catheters. But yet neonates and infants weighing less than 10 pounds, percutaneous central venipuncture is more difficult than in adults because of the small size of vessels and their characteristic ana-tomical relationship and the possibility of complications.
Since 1981, we have tried and achieved to place the catheter within the superior vena cava by supra-clavicular subclavian venipuncture in neonates and infants and the catheter is brou-ght out to the anterior chest wall through a subcutaneous tunnel which is made by using needle without any incisional wounds, for long term parenteral nutrition. Our -method is based on the experiences of supra-clavicular venipunctures in adult patients over one thousand cases from 1979, and studies of precise regional anatomical relationships of subclavian vein in seve-ral autopsy cases of neonates and infants.
